Traumatology and Orthopaedic Surgery

Traumatology and Orthopedic Surgery is the specialty that includes prevention, clinical assessment, diagnosis, medical and surgical treatment and follow-up until the final functional recovery of congenital, traumatic, infectious, tumorous, metabolic, or degenerative processes and congenital musculoskeletal or structural deformities and functional disorders, basically diseases of the musculoskeletal system (muscles, joints, ligaments, tendons, cartilage and bone).

In the broadest sense, Traumatology covers traumas anywhere on the body while, in general, although there are significant differences from country to country, Orthopaedics deals with non-traumatic musculoskeletal disorders, such as osteoarthritis and arthritis and diseases that affect the muscles, tendons and bone segments.
 
During childhood, before the musculoskeletal system has fully matured, Paediatric Orthopaedics addresses conditions that affect the growth and development of this system.
 
The Traumatology and Orthopaedic Surgery Services at Hospitales San Roque in Las Palmas de Gran Canaria operate 24-hours a day and have the capacity to deal with any osteo-tendon injuries that may occur.
 
The Orthopaedic Surgery Service includes:
  • An Arthroplasty Unit with proven experience in hip, knee and shoulder replacement surgery.

  • Growth factor and stem cell therapy, bone and joint infiltration in pathologies of the joints and bones where cell therapy has proven effective.
  • Ozone therapy, in which ozone infiltrations are made in  intra-articular and sublaminar or spinal trigger points.  
 
Bone tumours are rare lesions of the musculoskeletal system which can affect both children and adults. If they are diagnosed at a late stage, they may already have caused bone destruction, fractures, and have spread to near or distant parts of the body, especially in the case of malignant tumours or cancer; hence, the importance of recognising these pathologies as early as possible.  
Symptoms are varied, such as:
  • Pain, which can occur at night, after physical activity or even at rest.
  • Limited range of motion in a joint.
  • Persistent discomfort in children and adolescents with skeletal growth can raise suspicion of this disease.
When these symptoms appear, it is necessary to encourage patients to visit a specialist doctor to carry out an assessment and request complementary tests such as simple X-rays, magnetic resonance imaging or bone scintigraphy. Types of bone tumours: they can be benign or malignant. 
 
Malignant tumours are called sarcomas. This type of cancer requires, as in all these types of cases, joint management in collaboration with other specialties such as: radiotherapy and medical oncology, radiology, pathological anatomy, plastic and reconstructive surgery, nuclear medicine, rehabilitation and, of course, the help of an Orthopaedic Surgeon and Traumatologist with experience in this subspecialty. Bone cancer may also start in the blood; such is the case for lymphoma and myeloma, whose treatment is led by a haematologist who acts as team leader. Bone cancer must be treated with surgery when there is bone destruction with instability, or if the tumours affect neurovascular structures. 
 
There are also other malignant lesions in the bone that are secondary, -which means they come from other organs-, as in the case of kidney, lung, breast, and prostate cancer, although such lesions can come from any kind of diseased organ that releases these cancerous cells into the blood, which then colonise new locations, leading to metastasis.  It is very important to monitor the whole body in patients with this type of diseases, as early detection may also help prevent complications such as pathological fractures.
 
Nowadays, cancer is treatable even when presenting with metastasis. While in some cases the aim is to cure the patient, in other cases the focus is on improving pain management, reducing limitation, generating a feeling of comfort and autonomy, which is achieved mainly when patients do not need to use a stretcher or a wheelchair to move around, thus improving their quality of life.
